The Athletic Clearance for the 25-26 school year will not begin until the end of May 2025.

Please download the Online Sports Physical Packet and complete EVERYTHING in the packet. Be sure to complete all four courses on the 2nd page. All medical forms in the packet MUST be completed by your primary care physician or practioner. If you have any questions, please email Ms. Judski at judskicm@scps.k12.fl.us.

It takes up to five business days for paperwork to be approved so plan accordingly!
